Can anyone give me the link to the supplementary form from the NAS for claiming DLA,I used it 4 years ago and it was brilliant, I cant seem to find it now!!

As no-one has responded, and I don't know myself - might I suggest you try the NAS direct?

 

Alternatively, when I was completing DLA for my son, I got in touch with an independent benefits advice service which was run by my local authority. You might have something similar in your area. And you need to make sure it is INDEPENDENT advice you are getting and help with completing the DLA form.

I had a phone conversation today with the NAS's welfare adviser. She reckons I would score at most 6 for the PIP eligibility criteria (i.e. 4 for "engaging with other people face-to-face" and 2 for "making budgeting decisions") which falls below the 8-point threshold. My driving hangups do not count towards the mobility component, unfortunately.
