Hi everyone
Visit to the GP to day to update her and tell her that T was putting in a DLA claim and that she may be asked for a medical
report. So... on a previous visit i had asked about the cervical cancer vaccination and was told that if T wanted to have it
she would have to pay, because of her age which is 18 she will be 19 in four month's, and it would cost around 250
pounds, for the 3 course injection.
today i thought i would ask again, and it seems they are now given it to the student's who are in 6th form or 18 years of
age group, at your GP surgery. It just goes to show if you don't ask you don't get, her first injection is at the end of the
month.

HI
The NAS supplimentary form hmmm! there are a few things that dont apply to my daughter on the form example: I find it
hard to do up buttons/zips/tie up laces, the side effects of my tablets mean that...
Have people deleted what's not applicable to them before printing the form off.
Any advice would be much apreciated.

You must be joking right lol i'm in the middle of doing my daughter's she's 18 and it's my 1st time doing this form, i have always put it off. A: it was to painful to write about also i wanted to make sure my daughter wanted to share her personal problems, B: the form i find is a nightmare it's putting the write wording etc... I suppose once you have done it once and have a copy and it might be a bit easier. Well here's to hoping .

Hi Genna,
"My sons 26 and although he has A levels I don't think he would ever be able to fill in a DLA form, I did try but eventually filled it in myself and asked him to check it.
The supplimentary form from the NAS was a huge benefit, he found that easy to fill in and I used the information to fill in the DLA Form.
You can find the DLA application - supplementary information below. "

i have not had any support since my daughter was born and that's 18 years ago, no living parents, x partner was not supportive at all, and has lived in Japan for the last 7 yrs, so t has not seen her dad for 7 yrs.
I think you just have to keep picking yourself up and dusting yourself off, try and stay positive and get some time for you, some how Hope things improve for you.
