what do i put

you need to write a history of your child's development that highlights why he needs assessment

you can split it into medical, early development, educational (nursery onwards)

put into it all those difficulties he has had, all those quirks that indicate problems with social functioning etc

at the end you need to say what is currently being done for him in school and why you think this isn't enough/the right thing for him - the sentence 'we feel strongly that x needs a full statutory assessment in order to pinpoint exactly where his weaknesses and strengths are in order to provide the right sort of support for him in his learning so that he can overcome the difficulties he is facing in school' (or a variation on it) is pretty useful and one you can keep bringing up - if the staff working with him don't know precisely what his needs are then how can they make good judgements about what provision to make? how do they know who to ask for advice? .......

don't make it too long but make it as obvious as you can that his needs aren't being met without the assessment.

you will be asked for a 'parents' advice' as part of the assessment and this history will be a good starting point for that too.
