Health Questionaire for Uni

Recommended Posts

Hi everyone

As I am about to go and study nursing I have to get my GP to sign a health questionaire.I am worried because although I have no health concerns,I havent even been to the GP other than when I have been pregnant, the main thing they are looking for is proof of immunisation.I have had all my jabs but it was abroad so I got no proof.

I mentioned this on Fri at my pre induction, the woman couldnt really answer my question.Upon reading the forms I have seen they can check immunity, how do they do this???

Also how much does it cost and how long does it take for the GP to sign this? If anyone knows. Thank you in advance.

My brother had to do something similiar when he started his medical degree. He had to leave the forms with the receptionist and collect them the following day when his GP had signed them. I don't know if he had to pay, but the receptionist should be able to tell you the prices. You'd think the NHS should be fairly supportive of someone going into nursing though!

I can't help with the proof of immunity I'm afraid. Might the university fund the tests? If it's safe to be immunised twice, it might just be easier to have them done again.

Forms and letters generally don't count as NHS services so have to be paid for, although I don't know if there are standard charges or if health centres can make it up. My GP charges £20 for letters, I don't know if the same applies for forms; I always take forms to my hospital consultant / specialist nurses as they don't charge!

If your health centre has a website it might give their charges on there and also waiting times / procedures. If not you could call to find out. At my GP surgery, non-complicated cases don't even have to see the GP, the forms get given to reception, the docs complete them based on your records and you pick them up a few days later.

As for vaccinations, I'm not sure. I know there's some number you can call to find your immunisation record but I think this may only apply to the UK. Some immunisations it doesn't matter if you have them twice so they may decide it's safer to do this. Others I think they can tell with a blood test. It would probably be best to book an appointment with a practice nurse to discuss this.

Have just done a health ceck thingy for Ofsted and it cost me £30 to get it done by the GP, basically I completed most of it and the GP signed the back to say I was telling the truth about my hostory, left it with them for a couple of days

Not sure about the immunisation checks, I am sure I just wrote on my form I was up to date with them all

Thanks >:D<<'> We have to actually be screened to check we are immunised, mostly for HipB and in my case they may also want to check I have had BCG because of where I grew up.I will go and see them today,I dont mind getting one or two jabs again but hopefully not all of them :tearful:

£30 sounds like the right price, one of the tutors said it can be up to £70 :o

My surgery charges £25 for forms:( and it goes direct to the doctor who signs them, regardless of who you usually see!

an imunity check is usually just a quick blood test - should be free. I know they always checked my immunity to various things for each pregnancy.

Hi.

As regards the immunisations when I started nursing many moons ago [:lol: ] they just provided immunisations where they needed updating.

It is to cover the training institution as much as anything. They do not want you to catch hepatitis or german measles and pass it on to patients.

Karen.

Justine, sorry about my awful typos last night haha, was a tad tired

Anyways I have never had a BCG, I refused it at school as I was too scared and it has never affected my work choices etc. I know they are now only targetting specific areas for the BCG such as inner cities and ethnic groups (have no idea of your ethnicity), you may not need to prove or have that one
